Optimizely Forms
Optimizely Forms is a powerful tool that you can use to create web forms for event registrations, job applications, customer surveys, and so on. The submitted data is stored, and you can export the data in different formats.
Form element types
Optimizely provides default form element types that you can modify. The Forms gadget lists Basic and Action elements.
Create a form
To create a form on your website, you create a form container and add one or more form elements. The form elements can add, for example, a name field, an email field, and a Submit button to the form.
Configure a form element
The form elements are predefined building blocks for creating a form. You use the form elements as templates and drag and drop them into a form container, and then you can configure them to suit your needs.
Manage form submissions
This topic describes setting a retention policy and viewing, exporting, and deleting form submissions. Setting up a retention policy lets you control how long form submissions are stored in Optimizely Content Management System (CMS).

This section shows how to create a variety of different forms.
Example: Create an email sign up form
The following example shows how to create a single form to collect contact information from a visitor (name, email, and additional optional information). It uses the following form elements Text , Number , Text area , Hidden visitor profiling , Hidden predefined value , Reset button , and Submit button .
Example: Create a satisfaction survey form
The following example shows how to create a survey with satisfaction questions in multiple steps. The visitor selects answers from predefined choices.
Example: Create a customer satisfaction survey
This example shows how to create a customer satisfaction survey which displays different form elements depending on how the visitor answers. The survey is for a coffee shop that wants to know what their customers like and do not like about their services.
Example: Create a job application form
The following example shows multiple steps to create a job application form. In the first step, the visitor enters their name and email address and selects a job type to apply for.
