Optimizely Forms adds a block-based form builder to your CMS 13 website to collect visitor data and manage interaction. Create contact forms, newsletter sign-ups, event registrations, and job applications with reusable components and customizable rendering.

## Prerequisites

Optimizely Forms requires Optimizely CMS version 9 or higher. No additional license is required. For package and version details, go to [Apps platform compatibility](docId\:oGp5aaps96qkEIgnCuF_p).

## Install

Add Optimizely Forms to your project through the [EPiServer.Forms](https://nuget.optimizely.com/package/?id=EPiServer.Forms) NuGet package.
